The Case Management Team

Our aim is to provide you with a safe supported re-integration back into your community, and the role of the Case Manager is to support you and your family during your stay at NSIC to achieve this. 
    • We aim to ensure that, your rehabilitation programme is consistent throughout and driving towards a positive discharge for you. 
    • We will introduce ourselves once you are admitted in the wards usually within your first 10 days

We will endeavour to meet with you regularly, but if either yourself or family wish to make an appointment please do not hesitate to do so. 
We are situated opposite the spinal café in the Case Management Office.

From acute ward to rehabililtation

As you progress on the acute ward through to rehabilitation we will get to know you, your family and home circumstances, so that we can  identify where we can offer support and advice.

    • After you are admitted, we send a letter to your Clinical Commissioning Group (CCG) – this group is responsible for assisting in some aspects of  discharge.
    • As part of the multi-disciplinary team, we meet weekly to discuss your progress and are either involved in goal planning meetings or your Keyworker. 
    • We and the team set a discharge date at your second goal planning meeting, and over time gain a clearer idea of your possible care as time progresses.
    • We work on your behalf liaising with your local Social Service to enable them to assess your needs.
Nearer to discharge

Nearer to your discharge we will organise a discharge-planning meeting if required.

    • This provides the opportunity to meet with representatives from your local Social Services. 
    • The NSIC team will provide comprehensive reports to enable your local area to assess and understand your care needs.  
    • The services available in your local area will be explained to you. 
    • If you require care when you go home, we can liaise with the care agency and provide training at the NSIC.
Adaptations to your property, or rehousing

These can take time and therefore may go beyond the agreed discharge date for your rehabilitation.

    • In these circumstances, your case manager will work closely with those responsible for your ongoing community services to find an alternative suitable discharge destination.
    • This temporary placement is often referred to as an interim and could include options such as a residential or nursing home, or a stepdown facility local to you.

Your Case manager will look at other temporary solutions with you that can be used as interim option,
such as Aspire housing programme or private tenancy.

Legal advice about your injury

In some cases, legal advice about your injury might be needed. 

    • The NSIC has contact with solicitor firms who specialise in work with SCI – your case manager will provide detail of this.
    • Likewise, we have a visiting benefits advisor from the Department of Work and Pensions who can provide you with advice.
